Patient Forms

Once your new patient appointment is made, we will either text or email a link for new patient forms. The forms can be filled out from the link or printed and filled out. This will allow us to attend to your dental needs more quickly upon your arrival to our office.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
